Basically what the last scene was supposed to show us is that the Sopranos life is going to go as it did before the whole Phil thing got going. Carmela will continue to accept Tonys screwing other women and deal with the chance someday someone will cap him. If you watched the finale you realize that Tony looked at each person that came in. He has to wonder, "is one of these guys the one who kills me?". He will spend the rest of his life doing that. He will continue to struggle with depression and anxiety attacks. They did a good job building up the tension in my opinion. It was neat to watch Tony trying to have a family moment and watch the door at the same time.
