The radiation and fallout is what kills most people. As for N. Korea or Iran attacking us, they never will (atleast the continental US, I'm not arguing against regional troops). N. Korea will always poin the nukes at Japan or S. Kore, and Iran always at Israel or Iraq. Why piss on America directly when you can piss on America's friends a thousand times easier for nearly the same effect?
